Job Description
Position provides screening and navigation into therapy services. No community work, all in office.

Qualifications:
+ Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (or equivalent experience).
+ Prior experience working with individuals with severe mental illness or in a community-based setting preferred.
+ Strong communication skills, both written and verbal.
+ Ability to work independently and as part of a collaborative, interdisciplinary team.
+ Excellent problem-solving skills, with the ability to adapt to changing needs and circumstances.
+ Valid driver’s license and reliable transportation required (some travel within the community is expected).
