    Job Description

    The RN Case Manager role will be joining one of the largest hospice companies in Southern California. This is a very flexible role that allows an RN to get the best of bedside support coupled with interdisciplinary partnership with other clinicians. The role will be in the field all day every day Monday - Friday with 1 weekend on on call per month. There will be one day a week where you will go in to the corporate office 1X a week for team meeting other than that on the road.

     

    As the RNCM you will oversee a 15-20 patients/ caseload. You will be entering homes, nursing homes, assisted livings, to offer support and make sure care of plan is appropriate. Mileage is reimbursable after arrival at 1st visit.

    Skills and Requirements

    2 years of Hospice Nurse Experience with RN

     

    or** 1 year Acute Care Nurse Experience with BSN

     

    - Must be comfortable with administering IV medication and working with ventilators and pick lines Infusion or Dialysis Experience

     

    Wound Care Background

     

    Bilingual in English and Spanish
