Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible. Responsibilities MAJOR DUTIES: Maintains a current list of active participants in the program to enable tracking of service activities, utilization of services such as therapies, and resident satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services, including ensuring that the Community Care Request for Services (RFS) process if followed per National guidelines and timely. Mentors and trains a group of employees performing work at various GS levels up to their own grade and provides administrative, technical, and clinical guidance necessary for accomplishing the work of the unit. Carries out tasks at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer. Monitors and assures that monthly in-person assessments of Veterans in Contract Nursing Homes (CNH) are completed by the Social Work Program Coordinator and other designated employees, as required by the National CNH Program office. Ensures that the placement of Veterans into the CNH program meets VA eligibility requirements. Ensures that annual CNH facility assessments are completed as set forth in VA policy using knowledge of Centers for Medicare/Medicaid standards for CNH programs. Serves as the Contracting Officer Representative (COR) for the CNH program and ensures timely renewal of local/IDIQ contracts. Manages and tracks Veteran Care Agreements (VCAs), including ensuring timely renewal of existing VCAs. Completes ongoing market analysis, pursues VCAs with CNHs in high need geographic areas, when needed, and facilitates VCA application and approval. Monitors quality assurance related to these programs is within program parameters; leads monthly CNH Review IDT meetings and quarterly CNH Safety Meetings. Ensures the reporting and investigating of sentinel/adverse events as necessary. Prepares and reports to the appropriate facility oversight committee. Functions as the Lead for the CNH inspection team, conducting site visits for new contracts, annual facility assessments, and/or as follow-up to reports of care concerns. Ensure that surveys are conducted annually or as needed and ensures that the national CNH certification report is updated timely and accurately. Tracks Medicare Star Ratings and initiates waiver requests when needed and/or deferments and oversees monitoring of facilities on waiver and/or deferment. In conjunction with other program managers and Budget Analyst, manages the budget for the CNH program, tracking costs and projecting future needs. Provides administrative, technical, and educational consultation necessary for accomplishing the work of the program, including education to health care system staff and outreach efforts to community programs such as the state ombudsman program. Resolves informal and formal complaints and grievances. Performs a wide range of services and provides technical assistance to state, local, and private agencies, and universities based on monitoring activities and program reviews, and the development and analysis of information. Use sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Performs program analysis, development, review, research, design, and contact work with various agencies (public, DOD, state, etc.). Performs work related to the review, monitoring, analysis, planning and assessment of the CNH programs. Responsible for key project activities related to the planning, development, and implementation of the project component. Other duties as assigned. Basic Requirements: 1. Proficient in spoken and written English as required by 38 U.S.C. 7403(f) 2. Hold a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)? Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in social work. 3. Hold a licensed or certified by a state to independently practice social work at the master's degree level May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: 1. GS-12 Social Worker (Program Coordinator) Licensure: Licensed or certified at the advanced practice level, and able to provide supervision for licensure 2. Completed and passed the Association of Social Work Boards (ASWB) advanced generalist or clinical examination? 3. Possess one year of experience equivalent to the GS-11 grade level: Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Examples of specialized experience at the GS-11 include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. 4. Possess the following Knowledge, Skills and Abilities? Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
