Job Summary and Qualifications
The primary purpose of the Lead Social Worker is to ensure that primary operations of the Case Management Department function efficiently. This position maintains a caseload or equivalent assignment as volume dictates while providing a first line resource to Medical Social Workers. The Lead Social Worker may be called upon to act as a liaison for the medial and social issues that may arise and communicates information and recommendations to management as needed.
+ Performs ongoing monitoring of caseload for Social Workers based on appropriate level of care, utilization of services and plan of care. Ensures equitable caseload assignments.
+ Rounds with Social Workers over course of day as assigned by Manager/Director.
+ First line resource to Social Workers. Elevates issues as appropriate.
+ Monitors patient admit status and ensures timely intervention and reporting by Case Managers/ Social Workers.
+ Maintains a caseload or equivalent assignment as volume dictates.
+ When staffing necessitates will take full caseload/assignment.
+ Works with the social work team for education, accountability, elevation of professionalism in partnership with Director and Manager of Case Management.
+ Communicates potential/actual need for employee remediation to Manager and/or Director.
+ Performs initial screening of identified patients for appropriate level of care, priority, payer status and high risk criteria.
+ Collaborates with physicians and healthcare team in planning the care of patients and families within the acute setting and along the healthcare continuum.
+ Involves the physician, patient and family in planning care and the discharge plan.
+ Utilizes discharge planning rounds with physicians and care conferences with staff to plan and coordinate care.
+ Monitors plan of care to ensure timely and cost-effective use of resources.
+ Monitors patient’s progress along the plan of care and intervenes appropriately on variances when identified.
+ Coordinates with and corresponds to appropriate agencies and third-party payers regarding pre-admission screening, concurrent, outpatient and retrospective review process as it relates to federal and commercial insurance plans to maximize positive financial outcomes.
+ Regular, punctual and dependable attendance.
+ Assists manager in PI, Quality, and other studies/reports as needed. Collects and interprets data, reporting them to appropriate committees or persons.
+ In manager’s absence, assists Director in assigned duties and assigned meetings and acts as liaison between staff and administration. Ensures daily flow of department is maintained.
+ Assists manager with employee’s yearly evaluation, providing input into staff development.
+ Attends meetings and committees as assigned by manager.
+ Shares after hours call with Manager and Director.
+ Performs other related functions as assigned.
What qualifications you will need:
+ Masters Degree
+ Minimum two years’ experience in Medical Social Work required
+ 5 years Medical Social Work preferred
