We’re hiring a remote Triage Hospice Supervisor (RN) to join our growing hospice team! Flexible evening/night shifts (5p-8:30 or 10p-8:30a), every other weekend, and RN licensure in PA & NJ (or willingness to obtain) required. Make a meaningful impact in patient care—apply today!
Summary:
+ The Weekend Supervisor will serve in a leadership role by being responsible for the operational oversight during After Hours (evening, nights) for all agencies including Penn Medicine Hospice, Penn Medicine Home Health, Penn Home Palliative Care, and Penn Home Infusion Therapy. The Weekend Supervisor is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. They possess the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum. The Weekend Supervisor exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. They demonstrate within the competency domains of the Professional Practice Competency Domain: Continuous Quality Improvement, Evidence based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork.
Responsibilities:
+ Responsible for operational oversight during After Hours (evenings, nights) for all agencies including hospice, home care, palliative care and infusion. Operational oversight includes handling calls from patients and families and scheduling nursing and psychosocial visits as needed.
+ Manages the Visit and Call Schedule for hospice patients on the weekends, provides direction and support regarding changes in level of care, responds to service delivery concerns, and handles referrals to hospice.
+ Coordinates timely admissions to hospice along with procurement of DME, supplies and medications.
+ Independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from 18 years of age to the elderly. May be required to conduct patient visits to supplement staffing.
+ Seeks feedback in assuming leadership roles and provides feedback for improved clinical practice.
+ Demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system.
+ Complies with all regulations and standards of regulatory and accrediting bodies.
+ Works closely with the inter-professional health care team to facilitate the coordination of care across the continuum. The nurse will focus on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients.
+ Adheres to the Professional Practice Competency Domain.
Credentials:
+ Basic Cardiac Life Support (Required)

+ Registered Nurse - PA & NJ (Required)
Education or Equivalent Experience:
+ Bachelor of Arts or Science (Required)
+ 3+ years Nursing experience with specific focus on case management, home health, hospice, or infusion.
