Overview
Primarily responsible for the provision of spiritual care services to meet the social, emotional, and spiritual
needs of patients and families/caregivers, either directly or through coordination of care with other spiritual
counselors.

Responsibilities
+ Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual needs.
+ Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures.
+ Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team.
+ Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner.
+ Provide consultation, education, and support to the interdisciplinary group on spiritual care.
+ Meets measures as determined and required for job productivity and performance improvement.
+ Provide bereavement follow up services as requested.
+ Arrange for on-call availability of chaplaincy services.
+ Conduct or make arrangements for funeral or memorial services for patients as requested.
+ Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
Qualifications
Education:
+ Graduate of accredited school of theology, or related Bachelor’s degree.
+ 1 unit of CPE at time of hire. 2nd unit of CPE must be completed within 1 year of hire.
Experience:
+ Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body
Licensure:
+ Valid driver’s license when driving any vehicle for work-related reasons.
+ Valid Mandatory Reporter course completion by state(s) requirement
+ CPR Certification.
