PRN Hospice Nurse Practitioner

What You’ll Do as a Hospice Nurse Practitioner

In this role, you’ll provide expert hospice and palliative care to patients and families while ensuring comfort, dignity, and peace during life’s most meaningful journey.

You will:

+ Perform routine and emergency **face-to-face recertification visits** for patients entering their 3rd benefit period or beyond, including admissions, transfers, and extended stays.

+ **Assess, document, and communicate** clinical findings with the Medical Director, Attending Physician, and interdisciplinary team (IDG).

+ Provide **physical and emotional support** to patients and families, especially during crisis situations.

+ **Triage consults** and prioritize care based on patient acuity.

+ Deliver **bedside palliative care** and education for patients and caregivers.

+ Prevent unnecessary hospitalizations by **meeting patient needs at home** .

+ Maintain **accurate and compliant documentation** , billing, and coding per CMS guidelines.

+ Support quality care through **staff training, continuing education** , and clinical guideline development.

What You Bring to the Team

+ **Master’s Degree in Nursing** and national certification as a Nurse Practitioner

+ **Active state NP license** and ability to maintain licensure

+ **1+ year of clinical nursing experience** (hospice or palliative preferred)

+ Experience in **Family Medicine, Geriatrics, or Psychiatry** is a plus

+ **DEA certification** (if applicable for prescribing in your state)

+ **Valid driver’s license, insurance, and reliable transportation**

+ Ability to **travel up to 50%** , and lift/move up to 50–100 lbs as needed
