Organization: SCAH-Sutter Care at Home - Bay Position Overview: This position, in cooperation with Primary Care Physician, hospitalist and hospital staff, facilitates the Hospice admission process. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation. This position enters referral into electronic medical record and completes referral process and admits patient as appropriate. This position forwards hospital documents to hospice and communicates with hospice and hospital staff. This position provides education to hospital staff and physicians. This position is able to make home hospice visits to provide patient care coverage as the business need requires. **Job Description** : EDUCATION: + Graduate from an accredited school of nursing CERTIFICATION & LICENSURE: + RN-Registered Nurse of California + BLS-Basic Life Support Healthcare Provider + DL-Valid Drivers License + AUTO-Automobile Insurance TYPICAL EXPERIENCE: + 5 years recent relevant experience SKILLS AND KNOWLEDGE: + Must have clinical knowledge of disease entities common to hospice + Commitment to teamwork and customer service, both internally and externally, is required. + Experience with Microsoft office suite and electronic health record ly desired. + Collaborative and problem solving skills with customers and management. + Organizational skills. Must have telephone, interpersonal and written communication skills. Job Shift: Days Schedule: Full Time Shift Hours: 8 Days of the Week: Monday, Thursday, Tuesday, Wednesday Weekend Requirements: Sunday Benefits: Yes Unions: Yes Position Status: Non-Exempt Weekly Hours: 40 Employee Status: Regular Employees of Sutter Health and its entities may handle hazardous drugs in the course of their work, including patient care, which requires them to manage, store, prepare, receive, unpack, transport, dispose of, or administer drugs identified as hazardous or potentially hazardous by the National Institute for Occupational Safety and Health (NIOSH) and in accordance with the USP 800 guidelines. Pay Range is $72.04 to $96.80 / hour _The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
