Job Summary
This position is responsible for championing strategic initiatives in partnership with internal stakeholders and provider partners to meet or exceed targets and objectives. This role requires hands-on expertise in quality management, data analytics, workflow optimization, and clinical process improvement. The position functions in a matrixed environment, requiring collaboration across the organization. Is directly responsible for driving healthcare transformation across the ambulatory practices in quality, cost control, clinical quality improvement, and provider performance for those contracted in Medicare Advantage and value-based programs.
Essential Functions
+ Serves as the subject matter expert in HEDIS, risk revenue, value-based programs, practice transformation, incentive programs, ad Patient-Centered Medical Home (PCMH) programs.
+ In collaboration with medical office physicians and staff, designs and implements new workflow interventions and clinical practice interventions to achieve CMS and payer value-based reimbursement targets.
+ Responsible for overall direction of tracking progress toward milestones and deadlines. Escalates concerns to physicians and operational leadership in a timely manner.
+ Oversees the processes for working with program participants to obtain missing data to close quality gaps in care.
+ Provides support to practices around quality, data collection, reporting, and program initiatives.
+ Provides key data to physicians and practices to support continuous improvement in clinical quality initiatives.
+ Develops and implement a plan and tools to enable specialty practices are successful in the incentive-based programs.
+ Leads and fosters an environment for promoting sense of team and effective collaboration.
+ Identifies performance improvement opportunities and makes constructive suggestions within the team; assists with process improvement activities to improve workflow.
+ Facilitates ability to identify and share best practices as well as identify strategies and tactics that are ineffective.
+ Advises practice and administrative leaders of potential problems and recommends solutions.
+ Identifies and implements opportunities to improve operations and payer program deliverables.
+ Responsible for integrating new processes and best practices as requested by leadership.
+ Provides necessary education and training to practice physicians and staff, panel management, and operational teams related to quality requirements, EPIC workflows and skill gaps for practice transformation.
+ Confers with physicians, leadership and staff to effectively disseminate new policies, strategies, and tactics related to practice transformation and payer quality.
+ Manages practice transformation including practice transformation coordinators related to human resource activities, performance evaluations and work product.
Qualifications
Required
+ Bachelor's Degree health care administration or a related field
+ 2 years of relevant experience 2-4 years management experience
+ 3 years of relevant experience 3-5 years of Quality, Process Improvement, and Patient Experience in a healthcare related field with demonstrated data analytics, quantitative methods and reporting practices.
Preferred
+ Master's Degree
+ CRT-Professional in Patient Safety, Certified - IHI Institute for Healthcare Improvement
