When hospitals cannot provide the advanced care, expertise and new treatment options their patients require, they turn to us. *Job Description:* Provides holistic, quality and cost-effective nursing care for patients/families and/or others in the pre-hospitalization population within a unit, service or other primary area of practice; focuses on discharge planning which includes identification of risk factors and education. Applies the nursing process within the context of the organization's Framework for Professional Practice. Functions as clinically competent/expert. Delegates and supervises work appropriately. Reports to Director, Care Continuum. In this position the nurse will complete the Pre-Screening Call and complete preoperative chart reviews in Epic. During this call the nurse will provide holistic nursing care for patients/families/caregivers prior to hospitalization. The nurse will be responsible for accurately obtaining, assessing and identifying potential risks and barriers to expected outcomes. Patient care will be provided in partnership with a provider; including phlebotomy, EKG, and other preoperative testing as ordered. · Graduate of an accredited Nurse Practitioner or Physician Assistant Master’s program · Licensed as a Nurse Practitioner or Physician Assistant in the state of Connecticut · Board Certification as an Adult Nurse Practitioner (ANP), Family Nurse Practitioner (FNP), or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or certification as Physician Assistant · A minimum of 1 year practicing clinically in-person as an advanced practitioner is required **Job:** **Perioperative* **Organization:** **Hartford Hospital* **Title:** *Preadm Testing Ctr PATC (PER DIEM) - Nurse Practitioner (APRN) or Physician Assistant * **Location:** *Connecticut-Hartford-85 Jefferson Street Hartford (10068)* **Requisition ID:** *25163293*
