    The Medication History Technician plays a crucial role in patient admissions and medication reconciliation by obtaining and documenting accurate medication histories under the supervision of a licensed pharmacist for patients admitted to the hospital. This technician may also assist with other pharmacy technician duties.

In this specialized role, you’ll help ensure accurate medication histories are captured and documented—laying the groundwork for effective treatment and reducing the risk of medication-related errors._

    _What You’ll Do:_

    + **_Gather and document complete medication histories_** _for patients admitted to the hospital, under the supervision of a licensed pharmacist._

    + **_Support medication reconciliation efforts_** _to improve patient safety and reduce adverse drug events._

    + **_Assist with other pharmacy technician responsibilities_** _as needed, supporting overall pharmacy operations._

    + **_Contribute to patient progression_** _by ensuring timely and accurate medication information is available to care teams._

    + **_Collaborate with pharmacists, nurses, and providers_** _to ensure continuity of care and clear communication._

    _The successful incumbent in this position would be expected to work primarily evening shifts, and this position offers a schedule of 7 days on and 7 days off. Final scheduling accommodations will be determined by the hiring manager._

    Minimum Qualifications

    + Education: High School Graduate or Equivalent

    + Experience: 6 months minimum relevant experience required. Preferred experience with patient admissions and medication reconciliation by obtaining and documenting accurate medication histories under the supervision of a licensed pharmacist for patients admitted to the hospital.

    + Licensure/Certifications: Commonwealth of Virginia Pharmacy Technician Registration required. CPhT credential through National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB) required. For team members hired prior to February 1, 2020, certification as CPhT through NHA or PTCB is required by January 31, 2022.

    **Physical Demands –** Job requires standing for prolonged periods. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs.

    Onboarding Requirements

    Background checks and pre-employment health screenings** will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a **drug screen.

     

    Position Compensation Range: $20.68 - $32.05 Hourly
