Performs clerical and discharge support duties to assist the Network Care Management department.
The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position.
JOB DUTIES AND RESPONSIBILITIES:
+ Performs discharge support functions to assist the network Care Management departments with clerical functions as assigned.
+ Processes clinical information with designated vendor to obtain wound care equipment for discharge.
+ Responds to Medicare appeal inquiries for processing.
+ Maintains concurrent communication with care managers to provide timely updates related to all discharges processes.
+ Acts as a liaison between insurance companies and care managers.
+ May be required to deliver medical equipment, if onsite at a campus, to the unit Care Manager.
+ Acts as a liaison for vendors and post-acute providers both internal/external for the network.
+ May be required to cover some Lead liaison duties in absence of Lead; orientation required in advance.
+ Maintains confidentiality of all materials handled within the Network/Entity as well as proper release of information.
+ Complies with network and departmental policies regarding issues of employee, patient, and environmental safety and follows appropriate reporting requirements.
+ Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).
+ Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
+ Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes, and practices.
+ Complies with Network and departmental policies regarding attendance and dress code.

PHYSICAL AND SENSORY REQUIREMENTS:
Sitting for up to 5 hours per day, 1 ½ hours at a time. Standing for up to 2 hours per day, up to 30 minutes at a time. Walking for up to 30 minutes a day, up to 10 minutes at a time. Frequently uses fingers for typing and writing. Occasionally uses fingers, hands for turning/twisting. Rarely uses the hand for firm grasping. Frequently lifts, carries, pushes and pulls objects up to 10 pounds, occasionally pulls items up to 25 pounds. Frequently stoops, bends, occasionally crouches and reaches above shoulder level. Frequently kneels and squats. Must have the ability to touch as related to telephone and computer keyboard. Must have the ability to hear as it relates to normal conversations and high and low frequencies in person and on the telephone. Has ability for seeing as relates to general vision, peripheral vision and vision monotony. Able to speak clearly as related to communication on the telephone and in person.

Supervision provided by Supervisor Discharge Support
COMMUNICATIONS:
Excellent oral and written skills required to relate effectively with, patients, families, providers, the community, vendors, and other departments within the Network.
Must have strong written and verbal communications skills to perform assigned duties and responsibilities.
MINIMUM QUALIFICATIONS
EDUCATION:
Some college preferred. Health Care background preferred.
TRAINING AND EXPERIENCE:
Demonstrates comprehensive knowledge of computer skills.
Proficiency in Microsoft Office is essential.
WORK SCHEDULE: per diem. Minium of 1 shift per week if shifts are available after orientation. Must work at least 1 weekend per month.
