             Salary: $9 / Hour The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime. Minimum Requirements: + Current RN licensure in state practicing + At least one year of Case Management experience preferred + Current CPR if applicable + TB questionnaire, PPD or chest x-ray if applicable + Current Health certificate (per contract or stateregulation) + Must meet all federal, state and local requirements + Must be at least 18 years of age Registered Nurse (RN) Case Manager – Hospice Schedule: Monday–Friday, 8:00 AM–4:30 PM | On-call every other weekend Pay: $50/hour Mileage Reimbursement: $0.70/mile Position Overview: We are seeking a compassionate and skilled Registered Nurse to join our hospice team as a Case Manager. This role involves providing care, comfort, and support to terminally ill patients in both home and facility settings across Albemarle County. The RN Case Manager will manage a caseload of approximately 15 patients, conducting 4–5 visits per day, with travel up to one hour from the office. Key Responsibilities: + Deliver high-quality hospice care in accordance with nursing standards + Make clinical judgments and coordinate patient care with supervision + Maintain accountability for patient outcomes and assist in care management + Provide emotional support and education to patients and families + Document care accurately and timely About the Role: This is a field-based position requiring travel to patient homes and facilities. Candidates must be comfortable working independently, managing a dynamic caseload, and providing end-of-life care with empathy and professionalism.
