    Job Description

    An employer in the Southern CA is seeking a Patient Care Coordinator. This is a fully remote position, but the coordinator must sit in Southern California. This position supports the Case Management team. The coordinator is responsible gathering medical records, initiating member outreach to engage in case management, and coordinating with the RN Case Manager. Coordinator is responsible for mailing letters, educational materials, and community resources for the Member. The coordinator is also responsible for facilitating/coordinating patient appointments, benefits, and other healthcare needs.

    Skills and Requirements

     

    2+ years of experience in a Coordinator role

    Healthcare experience

    Strong communication and computer skills

     

    Experience working with EHR system
