             Job Description: The Manager of Care Management is responsible for overseeing the delivery of safe, high-quality patient care through clinical and non-clinical care management and care coordination in the ambulatory setting working with patients across the continuum of care. This role involves ongoing process evaluation and reviewing outcome metrics to ensure departmental performance and compliance with policies, procedures, patient safety initiatives, and regulatory requirements. The manager supports care management and care coordination program protocols and provides ongoing monitoring and process improvement to achieve departmental goals. Essential Functions + Facilitates the delivery of safe high-quality patient care through clinical and non-clinical care coordination, on-going process evaluation, and reviewing outcome metrics. Responsible for departmental performance and compliance with policies and procedures, patient safety initiatives, and contractual and regulatory requirements. Supports care management and care coordination program protocols and may provide ongoing monitoring and process improvement occurs to ensure goals are achieved. + Assists with developing specific departmental goals, standards, and objectives which directly support the strategic plan and vision of the organization. + Supports the development of proposals and business cases that deliver on strategic imperatives. + Responsible for supervising and coordinating the work of the care management staff, including assigning duties. Takes a role in professional growth of their team by providing ongoing education, creating a safe and efficient work environment. + Provides guidance, support, and opportunities for growth and development to help individuals reach their full potential as leaders and employees. This includes fostering a culture of mentorship, providing resources and training to enhance leadership skills, and actively supporting the professional growth and development of team members. + Accountable for KPI performance for portfolio using Intermountain model. Demonstrates ability to influence direct and non-direct reports to meet department goals. + Manages staff relations including performance management, staff satisfaction, and conflict management. Performs and oversees scheduling and recruitment. + Collaborates with internal and external physicians, clinical leadership, nursing leadership and teams to provide extraordinary care experiences for our patients and achieve clinical, operational, financial, and service goals. + Establishes and maintains effective working relationships with staff, members, network providers, and health plans. Ability to interact and communicate effectively with inpatient and outpatient staff, affiliated companies, providers, and local community personnel. + Collaborates internally to build and maintain healthcare management operating systems. + Participates in quality improvement initiatives, implementing evidence-based practices and monitoring patient outcomes. + Supports special projects and business analysis as requested. Degree will be verified. + RN License or Social Work License in state of practice + Three years of experience in hospital case care management, social work, or related area Preferred Qualifications + Master's degree in applicable field. + Management experience in related area Physical Requirements: Physical Requirements + Ongoing need for employees to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs. + Frequent interactions with providers, colleagues, customers, patients/clients, and visitors require employees to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately. + Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc. + May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts Location: Peaks Regional Office Work City: Broomfield Work State: Colorado Scheduled Weekly Hours: 40 The hourly range for this position is listed below.
