With this in mind, BayCare provides an array of benefits to help you meet the daily challenges of balancing all aspects of your life and career goals. + Medical benefits (Health, Dental, Vision) + Paid time off + Tuition reimbursement + 401k match and additional yearly contribution + Yearly performance appraisals and team award bonus + Extensive training and mentorship along with amazing career growth opportunities + Community discounts and more + **AND the Chance to be part of an amazing team and a great place to work!** **Care Coordination Coordinator** responsibilities: + Responsible for the daily Care Coordination activities related to the Care Coordination Assistants/Senior Assistants for all payors including but not limited to Managed Care, Commercial, Medicare, Medicaid, and Indigent. + Responsible for coordinating and assisting in development of processes within the Coordination area. + Assist the Manager in daily clerical functions including roll out of processes, gathering metrics for Team Map and other projects to improve Service, Outcome, and Cost. + Train new Central Intake Specialists. + Manage all EMR tables that Care Coordination is responsible to update as well as act as a liaison between Home Health and Pharmacy for updates in the EMR. + Assist Manager with staffing and scheduling including monthly calendars. + Fill in for absent team members as needed. + Be the first line of communication for the Assistants/Senior Assistants for questions on process. + Work with the Manager and will hold small group meetings in between staff meetings to keep team updated on changes and needs. + Serve as a resource for accurate ICD 10 coding, HCPCS and Medical Terminology.
