    **Work Schedule:** 8 Hr (8:30:00 AM - 5:00:00 PM)

    **Pay Range:** $25.20 - $39.69 per hour

    Summary:

    We are seeking an experienced and highly motivated** **Licensed Social Worker** **to be part of the Substance Use Intervention Team (SUIT) under the Department of Psychiatry at RUSH.

    Position Highlights** **:

    The SUIT (Substance Use Intervention Team) Social Worker is a critical member of our inpatient consult team. The SUIT Social Worker will provide inpatient clinical and case management support by assessing patient’s substance use related goals and barriers and engaging patients in motivational interviewing and care planning. The SUIT Social Worker will use their clinical evaluation to address the psychosocial needs of patients throughout their hospital stay and facilitate post discharge service coordination. Duties include performing psychosocial assessments, counseling & support, care coordination and disposition planning, and patient/staff & community communication and education in conjunction with the multidisciplinary SUIT team. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

    Required Job Qualifications:

    • Master's degree in Social Work.

    • Current LSW licensure in the State of Illinois.

    • Two years of experience providing social services in healthcare or community setting

    • Experience utilizing motivational interviewing and/or in a managed care environment.

    • Strong verbal and written communication skills, analytical skills and team-building skills.

    • Bilingual English/Spanish.

    • Ability to function independently, to organize and be self-motivated.

    • Ability to sense problems or potential problems.

    • Ability to apply inductive and deductive reasoning to information received and identifying relationships between seemingly unrelated events/information.

    Preferred Job Qualifications:

    • Current LCSW licensure in the State of Illinois.

    • Behavioral health experience.

    • Familiarity with electronic medical records.

    Responsibilities:

    1. Collaborates with team to identify patients in need of social work and behavioral health services.

    2. Reviews comprehensive risk assessments to develop plan of care and address barriers with the patient, patient's family, and care team members.

    3. Provides psychosocial education, supportive counseling, anticipatory guidance, and linkage to community resources during both individual and group sessions for patients and families as indicated by comprehensive assessment.

    4. Advocates within complex medical and community-based service systems to improve patient access to needed services.

    5. Leads collaboration with school behavioral health and social-emotional learning teams.

    6. Facilitates communication between patients, care team and community-based providers to ensure continuity of care.

    7. Contributes to development and documentation of individualized care for identified patients, focusing on specific psychosocial goals.

    8. Provides educational materials and resources to patient/family that are in a format which is specific to the patient/family learning abilities and language preference.

    9. Effectively collaborates with inter-professional healthcare teams within and outside of the Rush system.

    10. Works to improve identified CBP outcomes.

    11. Participates in quality improvement initiatives. Activities may include data collection, chart review, inter-professional collaboration, analysis of patient data, and care team meetings

    12. Attends individual and group supervision meetings for outpatient social workers as well as relevant social work trainings.

    13. Interviews, assesses and evaluates clients and families who present themselves for care; provides professional outpatient counseling and treatment with the goal of maximizing personal/family stability.

    14. Prepares intake summaries, treatment plans and case summaries while maintaining on-going confidential records.

    15. Charts member's treatment and progress in accordance with state and other regulations and in keeping with accepted standards.

    16. Develops, implements, and evaluates treatment approaches for clients and affected family members, including individual, family, group or intensive therapeutic modalities.

    17. Coordinates internal and external educational activities and training programs to support recruitment initiatives and goals of the education committee.

    18. Performs related duties as required.
