Advanced Practice Provider (APP) – Cardiology - Interventional Cardiology Support

About the Role

Catholic Health Physician Partners is seeking a dedicated and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing Cardiology practice in West Babylon, NY, supporting Dr. Ralph F. Caselnova, MD, an Interventional Cardiologist renowned for his patient-centered care and excellence in cardiovascular medicine.

The APP will play an integral role in providing comprehensive cardiovascular care, collaborating closely with Dr. Caselnova to deliver exceptional patient outcomes across both outpatient and hospital settings. About Dr. Ralph F. Caselnova, MD

Dr. Caselnova is a board-certified Interventional Cardiologist with over two decades of experience in comprehensive cardiac care. He is affiliated with Catholic Health Physician Partners and practices at 393 Sunrise Highway, West Babylon, NY, where he provides exceptional care to patients with coronary artery disease, heart failure, and other cardiovascular conditions.

Key Responsibilities

+ Provide direct patient care in collaboration with Dr. Caselnova, including patient assessments, follow-up visits, and care management for individuals with cardiovascular disease.

+ Support in-office evaluations, pre- and post-procedure visits, and chronic disease management (e.g., CAD, heart failure, hypertension, dyslipidemia).

+ Conduct diagnostic and therapeutic procedures as credentialed and appropriate to scope of practice.

+ Coordinate patient care transitions between inpatient, outpatient, and procedural settings.

+ Educate patients and families regarding treatment plans, medications, and lifestyle modifications.

+ Participate in quality improvement initiatives, patient education programs, and clinical documentation to ensure compliance and efficiency.

+ Collaborate with nursing, medical assistants, and administrative staff to ensure a seamless patient experience.

Qualifications

+ Master’s degree from an accredited Nurse Practitioner or Physician Assistant program required.

+ Current licensure in the State of New York as a Nurse Practitioner or Physician Assistant.

+ Certification in Adult-Gerontology Acute Care, Family Practice, or Physician Assistant Studies.

+ Cardiology or internal medicine experience preferred; new graduates with strong cardiology interest are encouraged to apply.

+ BLS and ACLS certifications required.

+ Excellent interpersonal, communication, and organizational skills.

Salary Range

USD $69.54 - USD $93.00 /Hr.
