Intensive Care Coordinator: $28/hr. or High Fidelity Wraparound Care Coordinator: $32/hr.
Amergis Healthcare Services is actively looking for a Intensive Care Coordinator and a High-fidelity Wraparound Care Coordinator who would be interested in a 13-week, Full-Time position at a facility in Chicago, IL. Additional details of the position are below:
Job Summary: The Care Coordinator takes primary responsibility for facilitating the care coordination process for children with a mental health diagnosis and their families. This includes leading Child and Family Team Meetings, collaborating with professionals across systems, and supporting the child in achieving individualized goals. The Care Coordinator helps families develop a positive vision for their future and empowers them through a strength-based model. This approach supports improved functioning for the child in the home, school, and community settings.
Case Load: Up to 12 or 18 (Depending on the severity of the cases)
Intensive Care Coordinator: High case load, Low severity
High-Fidelity Wraparound Care Coordinator: Low case load, High severity
Pay:
+ Intensive Care Coordinator: $28
+ High-Fidelity Wraparound Care Coordinator: $32
Site to Site Mileage Reimbursement: 70 cents per mile
Schedule:
+ Mon - Fri 9am - 5pm (Needs to be flexible)
+ Needs to be able to work a few hours in the evening or weekend 1 or 2 times a month
Travel Details:
- 60% in office, 40% on the road meeting with families in homes, public settings, or at schools
Duration: 13 weeks (Likely Extension/Possible Temp to Perm)
Job Duties:
- Perform outreach and engagement to locate, engage, and educate Pathways youth and their families. Outreach is required 3 times a week for 60 days or until the client is enrolled or declines services.
- Use a trauma-informed approach to effectively engage children/youth with significant behavioral health needs and their families/caregivers.
- Provide intensive care coordination using a strengths-based approach to safety planning, family team development, and wraparound planning to help maintain children in their homes, schools, and communities.
- Schedule, plan, and facilitate Child & Family Team Meetings.
- Build and maintain knowledge of available community resources and link youth and families to needed supports.
- Provide regular communication and close collaboration with multiple community partners.
- Use a system of care approach to assist families in coordinating services from community resources, placement providers, collateral agencies, the court, and other partners.
- Facilitate the creation of safety and crisis prevention plans.
- Collaborate with local MCR agencies when necessary.
- Enact the Care Coordination Model with each individual and family.
- Facilitate the application process and obtain consents for SFSP/FSP for eligible youth and their families.
- Provide care coordination services to SFSP/FSP eligible youth and their families.
- Help identify services and supports in the person-served community or natural environment.
- Complete service documentation in alignment with agency and program core performance standards.
- Prepare detailed documentation of activities, including opening/closing electronic records, completing assessments, updating Wraparound Plans, and maintaining Outlook calendar and correspondence.
- Provide a high level of customer service and client engagement.
- Demonstrate good writing skills for documentation purposes.
- Learn through in-person, virtual, and web-based trainings.
- Be organized, meet timelines, manage a caseload, and work independently.
- Possess strong interpersonal skills and the ability to collaborate with families, youth, and professionals.
- Maintain a caseload (e.g., 1:12 or 1:18 depending on population).
- Perform other related tasks as needed.
Requirements:
+ Bachelor's in a Human Service-related field
+ 2+ years of experience working with children and families
+ One-two years of experience managing large case loads
+ Experience with care coordination is a plus
+ Experience with Crisis Intervention
+ Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, and PowerPoint) and with other related software
+ Bilingual is preferred
+ Car + Valid Driver's License + Auto Insurance
+ Be able to float between locations and meet with families in their homes, public settings, and schools
