Minimum Qualifications
Possession of a current Louisiana Practical Nurse license, a temporary permit, or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state plus two years of experience as a licensed practical nurse.

Job Duties and Other Information
Job Duties:
The outpatient LPN-3 position serves as a Nursing Care Coordinator, whose duties include but are not limited to:
+ Providing treatment and care of persons with acute and chronic medical and behavioral health conditions.
+ Responsible for performing individualized direct client care.
+ Responsible for post-hospitalization client navigation.
+ Working with clients, families, and outside agencies to explore community resources to assist with managing healthcare for clients.
+ Assisting the Behavioral Health Clinics in administering HIV tests and does pre- and post-counseling to clients.
+ Documenting in the electronic health record (EHR) all nursing services provided.
This position is domiciled at the Rosenblum Behavioral Health Clinic in Hammond, but may travel to other clinics throughout the FPHSA catchment area. The incumbent of this position must maintain HIV/AIDS training certification in HIV Prevention Counseling required by the Louisiana Office of Public Health’s HIV/AID Program.

Compensation: This position is eligible for premium pay of $1.00/hour for hours worked performing direct patient care.
The following rates are Special Entrance Rates (SER) established for this agency in this job title:
