    Logan Health's Brendan House, located in Kalispell, MT, is looking for a part-time BSW to join their compassionate team as a Medical Social Services Advocate!

     

    For 40 years, Logan Health’s Brendan House has been the cornerstone of compassionate care in the Flathead Valley, providing skilled nursing, rehabilitative services and long-term care with a commitment to dignity and respect. Our facility is attached to Logan Health Medical Center so you can enjoy all it's amenities such as cafeteria, coffee shop, and on-campus fitness center.

    What you'll be doing:

    The Medical Social Services Advocate will primarily be serving Brendan House's Long Term Care patients and may participate in the following:

    + Perform psychosocial assessments, complete PHQ-9 questionnaires, and BINS screenings.

    + Assist with Medicaid application starts and redeterminations

    + Attend Interdisciplinary Team care conferences, IDT meetings, as well as meet with residents and their families.

    + Offer compassionate end-of-life support to residents and families, ensuring comfort, dignity.

    + May help with some discharge planning or referrals as needed.

    + Confident in your ability to complete assessments, advocate for residents, and connect them with needed resources.

    Qualifications:

    + Bachelor’s Degree in Social Work, Social Services field required.

    + Experience in a medical setting, skilled nursing, or long term care is preferred.

    + Current CPR per assigned area(s) may be required.

    + Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

    + Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

    + Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

    + Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

    Other Job Specific Duties:

    + Addresses identified support and social needs of patients including, but not limited to: trauma, palliative care, comfort care/end of life patients, patients with substance use disorder, patients with complex social situations, patients with active psychiatric and/or behavioral concerns, situations with Guardianship and other legal issues impacting health and recovery.

    + Partners with appropriate stakeholders across the healthcare continuum.

    + Explores and communicates solutions and recommendations to social barriers impacting healthcare with appropriate stakeholders. Seeks input from the multidisciplinary team. Completes thorough and accurate documentation in the patient’s medical record.

    + Performs as an active member of the healthcare team making early referrals and communicating thoroughly with post-acute partners, family members, and patients; Operates from a person-in-environment framework and strength-based perspective; Exhibits strong, professional communication skills and ensures available community connections are made.

    + Exhibits knowledge and skills necessary to provide for the physical, psychological, and cultural needs of the patient. Advocates for clients' rights to self-determination, confidentiality, and access to supportive services and resources, and appropriate inclusion in informed decision-making that affects health and well-being.

    + Consistently reflects ownership and professionalism towards Care Coordination. Identifies areas in need of improvement and serves an active role in developing solutions. Exhibits the Organization’s Core Values to both internal and external customers.

    + Responds to, and provides direction in, a changing workload in a positive and professional manner.

    Shift:

    Day Shift - Variable Hours (United States of America)

     

    Schedule: Part-time, 20 hours per week. This position will primarily be working two 8-hour shifts Tuesday and Friday, and a 4-hour shift on Thursday.

     

    Shifts may be subject to change depending on staffing, coverage, or needs of business.

     

    Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
