    Description

    A Typical Day for a Performance Improvement Partner with** **WesleyLife at Home:

    + In collaboration with our Director of Quality and Hospice Team the Performance Improvement Partner collects, compiles, and uses data to identify outcomes, documentation, compliance, and performance trends.

    + Coordinates monthly QA meetings. Participates in case conference activities and ensures conferences are conducted and documented according to policy through attendance and record reviews.

    + Works collaboratively with Case Manager RNs to monitor admission and discharge records for timeliness, appropriate levels of care, completeness and accuracy of documentation, fulfillment of physician orders, and completeness of medical records.

    + Supports and educates around the HOPE tool along with project monitoring.

    + Participates in an on-call rotation for clinical support.

    + **Current Openings:** Full-Time Monday-Friday // This is role is in-person in a positive and collaborative office environment.

    Compensation:

    + The pay range for this position is $75,025-$90,000 / year. The starting pay rate will be based on years of experience.

    + Mileage Reimbursement at 58.5 cents per mile

    Qualifications

    Licenses & Certifications

    Required

    + Registered Nurse

    Experience

    Required

    + Valid driver’s license, reliable transportation, valid insurance, and clean driving record

    + Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen

    + Active RN license in the state of Iowa

    + Minimum of 2 years Hospice experience

    + A minimum of 2 years of nursing experience is required
