    Overview

    **Purpose Statement/Position Summary:** The Social Services Casework is under the supervision of a Master's level social worker, providing specific services to patients and their families. The Caseworker will provide non-clinical, peer-to-peer support and guidance to families of children with congenital heart disease (CHD) or other complex newborn conditions requiring extended hospitalization and ongoing care. Drawing upon their personal "lived experience" as a primary caregiver of a child with complex health needs, this specialist will act as a mentor, advocate, and liaison, helping new families navigate the healthcare system, access resources, and cope with the emotional and practical challenges of their journey.

    Essential Duties and Responsibilities

    + **Peer Support and Mentorship** : Establish and maintain a trusting, non-judgmental relationship with families, sharing insights from personal experience to provide validation, hope, and emotional support.

    + **System Navigation & Advocacy** : Assist families in navigating complex medical, educational, and social service systems. Coach parents on self-advocacy skills, ensuring their voices and preferences are heard within the care team.

    + **Resource Connection** : Identify and connect families with relevant community resources, support groups, and financial/insurance information. Develop and maintain a directory of current, vetted resources.

    + **Education & Information** : Help re-state clinical information into everyday language for families and assist them in understanding their child's diagnosis and treatment plan, without providing medical advice.

    + **Support Group Facilitation** : Coordinate and facilitate support groups for parents and caregivers, offering a safe space for shared experiences and mutual support.

    + **Collaboration with Care Team** : Function as an integral member of the interdisciplinary team (physicians, nurses, social workers, child life specialists), communicating family-identified needs and ensuring a family-centered care approach.

    + **Crisis Planning Support** : Assist families with crisis and safety planning by leveraging personal experience with stressful situations and hospitalization.

    + **Documentation** : Maintain accurate and confidential records of interactions and services provided, adhering to HIPAA and other regulatory requirements.

    **Minimum Qualifications/Work Experience:** 1+ year social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience. *Billingual skills may be required depending on assignment and patient population.

    **Education/Licensure/Certifications:** Bachelor's degree in social work or an equivalent combination of related education and work experience required.

     

    American Health Association BLS certification required.

     

    Preferred: Master’s degree in Social Work (MSW) or an equivalent combination of related education and work experience with LCSW or Associate Clinical Social Worker registration.

     

    Pay Scale Information

     

    $51,979.00-$85,394.00

Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
