Participates in the development of palliative care practice standards for nursing 1. Assumes leadership position in practice related committees as appropriate 1. Evaluates effectiveness of the program and plans for future growth 1. Works to identify budgetary requirements of the program + Contributes to the development and implementation of palliative care pathways/care maps and policies and procedures, integrating elements for enhanced quality, service and resource management 1. Seeks professional and personal development opportunities through continuing education 1. Generates education opportunities for staff development concerning healthcare in informal and formal methods 1. Guides multi-disciplinary team in developing an individualized plan of patient care, which reflects acute care needs, symptom management, palliative and supportive care, discharge needs, and patient/family education + Collaborates with all members of the healthcare team in planning, implementing, coordinating, monitoring and evaluating care delivery for the palliative care patient in a variety of settings. 1. Servesas aconsultant based on specialized expertise for healthcare team members and various patient and community populations 1. Utilizes verbal and written communication strategies to ensure continuity of care (i.e., care conferences, medical record entry, etc.) 1. Provides counseling and clinical intervention based on palliative care expertise in various settings along the continuum of care 1. Participates in clinical rounds + Responds to palliative care consultation requests. Actively assesses, plans, implements, coordinates, and evaluates the care of hospitalized patients with advanced care needs. Makes recommendations about the patient’s palliative care needs and pain and symptom management: 1. Collaborates with multidisciplinary healthcare team 1. Consults additional resources as appropriate (i.e. pharmacy, speech therapy, wound care, etc) 1. Communicates recommendations to primary care physician and/or attending physician, consultants as appropriate, nursing staff, and the patient and their family members 1. Evaluates effectiveness of interventions and makes adjustments/recommendations as appropriate 1. Provides support and education to the patient and family 1. Coordinates family conferences among the patient, family, and healthcare team to discuss goals of care 1. Follows patients through/across departmental lines as appropriate to that individual’s care needs + Demonstrates competence in performing as an active member of the healthcare team; managing acute and chronic health care needs of the adult/geriatric patient population 1. Formulates a plan of care in collaboration with physicians, other healthcare team members and the patient/family 1. Manages therapeutic regimens as outlined by physician orders, established protocols and pathways 1. Facilitates patient and family education in individual or group setting utilizing appropriate teaching materials and adult learning strategies 1. Evaluates achievement of individual patient goals, tracks patient variances individually and in aggregate for a population and recommends actions to improve patient progress toward goals 1. Provides services to promote continuity of care 1. Assists in the triage of patient calls, scheduling and follow-up of patient activities, as necessary 1. Emphasizes health promotion and disease prevention per protocols and individual patient needs + Organizes process improvement and multi-disciplinary clinical outcome activities utilizing nursing expertise 1. Identifies variance in care delivery processes based on systematic and empirical evidence that impacts the palliative care patient population 1. Develops a plan of care for individual patients and support systems that meets the client’s healthcare needs and utilizes the most appropriate resources to meet both clinical and fiscal goals 1. Coordinates a plan that results in reduced average length of stay for chronically and/or seriously ill palliative care patients 1. Participates in research activities in compliance with the organization’s investigational review board and regulatory agencies 1. Maintains accountability for accurate and timely data collection and assists in analyzing and interpreting outcome findings using support services, as needed + Initiates plan of care to meet specific needs of palliative care patients 1. Assists patient and family members in grieving process and with making end-of-life decisions 1. Improve patient outcomes by facilitating early interventions for common geriatric problems: skin breakdown, dehydration, incontinence, falls, confusion/delirium, malnutrition, sleep disorders, pain 1. Maximizes functional abilities and assists in maintaining autonomy of patient 1. Increased needs for recovery time between activities What qualifications you will need: + Advanced Cardiac Life Spt must be obtained within 30 days of employment start date + Basic Cardiac Life Support must be obtained within 30 days of employment start date + (RN) Registered Nurse + Bachelors Degree Medical City McKinney opened its doors more than 90 years ago.
