    Job Summary and Qualifications

     

    Under the general direction of VP of Neuroscience, the Neuroscience Program Manager is responsible for providing coordination and development of comprehensive stroke and neurological care.

     

    **What you will do in this role** :

    + Demonstrate knowledge of human growth and development, assessment, range of treatment, and care of low and high-risk neurological patients.

    + Coordinate care for the neurological patient across the continuum, including stroke screening, education, follow up phone calls, support groups, and community outreach.

    + Work collaboratively with EMS and post-hospital partners to improve stroke protocols and programs.

    + Promote patient experience initiatives and provides a seamless positive patient experience.

    + Make daily clinical rounds to ensure compliance with core measures and evidence-based care on a concurrent basis.

    + Maintain a comprehensive stroke log and ensure accurate and timely data submission to stroke or other neuroscience related registries.

    + Work collaboratively with quality management to collect, aggregate, and analyze data for the improvement of patient outcomes. Present aggregate data in an organized manner at stroke committee/team meetings. Effectively manage corrective action plans as needed.

    + Collaborate with nursing and physician leadership to provide real-time and structured education for staff.

    + Assist in navigating patients/families through the acute hospitalization and transitions of care, keeping them informed and educated appropriately.

    + Lead the stroke program with the multidisciplinary team (PT, OT, SP, pharmacy, social worker, case manager, home care agency or other team members) for optimal patient outcomes.

    + Provide oversight for Joint Commission disease-specific certifications.

    + Provide support for growth of the neuroscience product line through program development.

    + Provide leadership and educational support for the clinical staff on stroke units.

    What qualifications you will need:

    + Graduate of a accredited school of nursing required.

    + Current licensure as a Registered Nurse (RN) in the state of Tennessee or compact state required.

    + 2 years of clinical experience in critical care and/or emergency services required.

    + Current NIHSS required.

    + Prior leadership and/or stoke program coordinator experience preferred.

     

    TriStar Skyline Medical Center is a 350+ bed facility. We are Tennessee's first comprehensive stroke center and a national leader in neuroscience. In addition, we are equipped with 3 helipads, new ICU and Medical/Surgical floors, and burn care services. We offer a CARF accredited Inpatient Rehabilitation program and are a state-designated Level I Trauma Center. As an Accredited Chest Pain Center with PCI, TriStar Skyline is a leading provider of emergency heart care.
