             Employment Type: Part time Shift: Description: Social Worker Emergency Department, Mount Carmel Dublin Hospital Part-time Position Purpose: The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from Multi-disciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. Specific age groups that are served by this position are circled. **This position will work Thursday and Friday; 8 pm - 8:00 am. Minimum Requirements: Job Qualifications (Knowledge, Skills, and Abilities) • Education: Masters Degree in Social Work • Licensure / Certification: Current LSW license to practice as a social worker in the State of Ohio • Experience: Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting. • Effective Communication Skills • Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements. • Possesses excellent clinical assessment and analytical skills. • computer competency • Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations. • Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care • Understanding of pre-acute and post-aute venues of care and post-acute community resources. • Ability to work independently and exercise sound judgement in interactions with physicians,payers, colleagues and patient and their families. • maintains a professional demeanor Position Highlights and Benefits: Competitive compensation and benefits packages including medical, dental, and vision with coverage starting on day one.
