Job Summary and Responsibilities

+ Provide the physician and the health care team with recommendations and information which will assist in developing an overall plan for the patient’s care upon consultation.

+ Partner with providers and hospital staff in effective discharge planning.

+ Educates patients and families by providing facts about diagnosis, treatment plans and available community resources.

+ Collaborates with the physician(s) and health care team and serves as a patient advocate and liaison with community agencies.

+ Develops and maintains good working relationships with community treatment network to ensure the availability of quality services.

+ Coordinates care with other health care community providers. Connects patients and families with these providers/services.

Job Requirements

+ Bachelor’s degree in Social Work or Human Services related field, preferred

+ Acute care experience as a social worker preferred

Pay Range

$22.51 - $31.79 /hour
