Job Description
We are seeking a dedicated RN Case Manager to perform telephonic case management for Medicare Advantage members in the state of Florida. This role involves care coordination activities, developing care plans and assessments, conducting new referrals, calling members, and building new cases in Athena EMR.
Responsibilities
+ Perform telephonic case management for Medicare Advantage members.
+ Coordinate care activities and develop comprehensive care plans and assessments.
+ Conduct new referrals and engage with members through calls.
+ Build and manage new cases using Athena EMR.
Essential Skills
+ Valid RN license in Florida.
+ Minimum of 2 years of case management experience in hospital, acute care, home health, or hospice settings.
+ Proficiency with Athena EMR software.
+ Strong computer skills, including Microsoft Suites, Excel, data entry, and computer documentation.
Additional Skills & Qualifications
+ Ability to manage a caseload of at least 73.
+ Experience in patient care, triage, medical records, customer service, and utilization management.
+ Experience in fast-paced and high acuity nursing environments.
Work Environment
This is a hybrid opportunity with work hours from Monday to Friday, 8am to 5pm. The role requires a desire to focus solely on case management work. This is a Contract position based out of Orlando, Florida.
Pay and Benefits
The pay range for this position is $35.00 - $45.00/hr.
