    Description We are looking for a dedicated and compassionate Referral Coordinator to join our team in Los Angeles, California. In this long-term contract position, you will play a pivotal role in guiding patients through the healthcare system, ensuring they receive access to necessary specialty services and resources. Your goal will be to provide education, support, and coordination to enhance patient outcomes and streamline their healthcare journey.

    Responsibilities:

    • Coordinate referrals to specialty care, diagnostic services, and mammogram appointments, working closely with external providers and health plans to ensure timely scheduling.

    • Notify patients of their appointment details, explain the importance of attending specialty or diagnostic visits, and provide reminders to encourage follow-through.

    • Maintain detailed records of referral statuses, including appointment schedules, patient notifications, and provider consultation outcomes.

    • Prepare and submit monthly reports outlining referral activity, patient outcomes, and other relevant metrics.

    • Research and update information on local specialty and diagnostic service providers to ensure accurate and current referral options.

    • Collaborate with Patient Navigators across service sites to share information and improve patient access to resources.

    • Serve as a liaison between patients and medical staff, ensuring clear communication for both internal and external healthcare services.

    • Educate patients about available healthcare benefits and connect them with Eligibility Specialists to facilitate enrollment.

    • Assist in organizing outreach efforts for health screenings, including mammograms and Pap smears, while providing education on breast health awareness.

    • Review and manage patient charts and forms to ensure completion and accuracy, replenishing necessary materials as needed. Requirements • Proficiency in basic medical terminology to communicate effectively with healthcare professionals.

    • Knowledge of medical insurance processes, including verification and eligibility.

    • Experience using electronic medical record systems to document patient information and manage referrals.

    • Ability to schedule patient appointments efficiently and accurately.

    • Strong organizational skills to track referral statuses and maintain detailed records.

    • Excellent communication skills to educate patients and collaborate with healthcare providers.

    • Familiarity with the Patient-Centered Medical Home model and its care principles.
