Job Details The Psychosocial and Bereavement Manager is responsible for the oversight of Hospice Social Work, Bereavement and Volunteer services. This key position ensures the delivery of care in accordance with Department of Health, Joint Commission and Federal standards and regulations. Working closely with the Director of Patient, serves as a resource to patients, families and clinical staff to monitor the overall effectiveness of services provided. + Responsible for the clinical supervision and coordination of all activities of Social Work, Bereavement and Volunteer services in both the Community and Inpatient settings. + Oversees patient assignments and develops departmental goals consistent with and supportive of Catholic Health’s mission, vision and values. + Oversight of Bereavement Services, including Bereavement support groups, Bereavement Mailings, Camp Hope, Memorial Services, Bereavement Grants, and Workshops. Fields community requests for information and referral for group support and other services, as necessary. + Creatively identifies and develops enhancements to the Social Work and Bereavement Care program, enhancing processes, quality of care, program deliverance and work flow. + Coordinates with external partners and volunteer organizations to expand the program offerings. + Work in close collaboration with interdisciplinary teams, Nurses and Chaplains, to ensure comprehensive support for patients and families. + Assists the Director of Patient Services in preparing, managing, and adhering to Psychosocial and Bereavement program budget and management of any grants specifically designated for these services. + Ensures the documentation of Psychosocial and Bereavement assessments and interventions and tracks quality of care and productivity. The successful candidate must meet the following minimum requirements: + Master’s Degree in Social Work with a current and valid New York State license to practice as a Licensed Clinical Social Worker (LCSW required). + A minimum of two year of supervised work experience in a healthcare or related setting, preferably in bereavement/hospice care + A thorough understanding of the psychological, emotional, cognitive, medical, social and behavioral sequel of grief as well as professional training and prior clinical experience in providing individual, couples, family and group counseling. Familiarity with the hospice concept and its implementation + Excellent organizational skills and attention to detail. Must be able to manage multiple priorities simultaneously + Excellent communication and customer service skills + Proficiency in Electronic Medical Records (EPIC a plus) and Microsoft Office products Salary Range USD $110,000.00 - USD $130,000.00 /Yr.
