We are seeking Licensed Practical Nurses to support both our residential and day program settings.
POSITION DUTIES:
+ Administers general nursing care, vaccinations, wound and patient care to Health Center patients under the supervision of a Registered Nurse.
+ Follows the direction and guidance of the Charge Nurse in providing and coordinating patient care.
+ Greets patients and their family members/caregivers with a positive and encouraging attitude.
+ Performs patient vital signs, ECG, phlebotomy and other patient tests as needed.
+ Obtains patient history, and reconciles accurate medication lists and any information the physician/consultant provider requests, and records them into the patients’ Electronic Health Record.
+ Ensures lab reports, radiology and diagnostic reports are attached to the patients’ chart/paperwork at the time of visit.
+ Communicates effectively with staff members, patients, nurses, pharmacies, diagnostic and treatment centers, and other physician offices in order to ensure the continuity of patient care.
+ Assists physicians/consultant providers with medical procedures and treatments, and examines all documents and the Electronic Health Record (EHR) for accuracy and completeness.
+ Records the administration of vaccines, diagnostic tests, and any other patient information accurately and timely into the EHR.
+ Tracks, verifies and documents the receipt/completion of outside patient referrals.
+ Records and ensures appropriate follow-up of critical/abnormal tests and lab results.
+ Ensures timely transmittal and faxing of Electronic and paper prescriptions.
+ Notifies the appropriate person of patient medication changes and/or special plan of care instructions.
+ Attends meetings/ in-services as required by Health Center policies and as needed.
+ Collaborates with the health care team to ensure proper patient care and coordination.
+ Communicates effectively and uses appropriate judgment when making decisions.
+ Obtains patient Prior Authorizations for medications, diagnostic tests and services as needed.
+ Monitors vaccine expiration dates and medical supplies for stock, cleanliness and working order and notifies the Charge Nurse as needed.
+ Assists the physicians/provider consultants with the EHR, monitors patient progress notes for completeness and notifies the provider to close-out (lock) EHR notes at the end of clinic sessions.
+ Maintains patient privacy and confidentiality by abiding by HIPAA rules and regulations.
+ Performs all other duties as assigned.
REQUIREMENTS:
+ High School Diploma or equivalent.
+ Graduate and licensed to practice as a Practical Nurse in New York State.
+ Ability to communicate effectively.
+ BCLS Certification.
+ Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business contact, the NYS and federal false claims acts, whistle blower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all compliance investigations and reviews.
