             Description: Summary: Ensures optimal coordination of patient access and care. Maintains customer satisfaction. Relays patient results with letters and/or phone calls, sets up follow-up imaging exams for patients, sets up appointments with the breast care clinic if the radiologist recommends short term follow-up. Works in collaboration with the radiologists, leadership, and clinical caregivers. Identifies opportunities to increase patient volume and services while continuing to control expenses in the corporation. Responsibilities: • Assists in maintaining a clean, safe and ordering work environment • Identifies potential locations for van screenings with potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services • Analyze community needs to determine program opportunities • Tracks and reports data to substantiate efforts • Provides informal education to patients. Assists patients with utilizing available resources, including cancer services and other breast related appointments. • Develops and maintains a strong working relationship with community agencies and internal clinical staff. • Maintains accurate records by documenting all contacts, services provided, and outcome in order to track all patients and services for future reference. • Maintains event calendar • Own Patient Experience; making sure that patients, providers, and clinic staff are aware of expectations • Calls patients directly to explain and schedule additional imaging • Guides patients through the health care system, help patients be prepared and arrive at scheduled appointments • Provides guidance on scheduling to support staff and volunteers for outreach and educational events • Obtains previous mammography studies from outside facilities and ensures they are available for radiologist interpretation in PACS. • Assist with participation in educational events promoting breast care and community outreach. Stays current of health care topics that may interest the community • Demonstrates good judgement and planning when responding to requests or questions while interacting with physicians, patients and family members. • Follow hospital safety guidelines • Follow all hospital policies and procedures; including infection control and safety procedures • Utilizes behavioral standards as the basis for decision making and to facilitate the department and hospital mission • Assumes personal responsibility for ongoing professional development, in-services, and accepting new responsibilities • Performs duties in an independent manner with little or no direct supervision • Observes hospital dress code • Demonstrates a willingness to accept non-routine work assignments, as appropriate • Performs other duties as needed Education, License & Certification: None required Experience: One year in mammography helpful Essential Functions: • Attends management meetings and participates in other hospital committees as required. Complies with Guthrie expectations, such as regular attendance at meetings, safety training, annual diversity training, etc. • Work effectively and provide direct communication with the radiologists, clinical care team, community vendors to ensure patient care and services • Ensures that practices provide service/care that meets or exceeds patient care standards Complexity of Work: Within the scope of the job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision.
