Summary:
The Home Infusion Pharmacist is responsible for overseeing the operational and clinical dispensing workflow of the UNC Health Home Infusion Pharmacy. This role ensures accurate order verification, compounding oversight, quality assurance, and delivery coordination for home infusion therapies. The pharmacist collaborates with prescribers, nursing, supply chain, and billing teams to support safe, compliant, and cost-effective infusion services.
Responsibilities:
+ Verify, interpret, and process physician orders for home infusion therapies (e.g., TPN, antibiotics, chemotherapy, immunoglobulin, hydration, and inotropes).
+ Provide clinical and operational supervision of the sterile compounding area, ensuring adherence to USP <797>/<800> and internal policies.
+ Oversee daily workflow, including batching, packaging, courier coordination, and medication labeling for home delivery.
+ Support supply chain management, including assessment of drug shortages, product substitutions, and communication with wholesalers and clinical teams regarding alternatives.
+ Ensure all compounded products meet beyond-use dating, labeling, and documentation standards before release.
+ Participate in daily operations huddles, staff training, and continuous quality improvement initiatives.
+ Monitor compliance with Joint Commission, DHSR, and NC Board of Pharmacy requirements.
+ Support emergency preparedness and continuity of operations planning related to supply chain interruptions or courier delays.
+ Collaborate with Revenue Cycle, Intake, and Nursing to ensure order completeness, accurate documentation, and appropriate billing.
+ Serve as pharmacist-in-charge or acting manager when designated.
+ Provide mentorship to technicians, students, and residents.

Education Requirements:
• PharmD or BS Pharmacy Degree
Licensure/Certification Requirements:
• Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
Professional Experience Requirements:
• No prior experience required.
Knowledge/Skills/and Abilities Requirements:
• Expertise in sterile compounding standards (USP <797>/<800>), medication stability, and infusion device compatibility.
• Strong organizational and problem-solving skills, including management of drug shortages and supply chain disruptions.
• Proficiency in EMR and pharmacy systems (EPIC, Brightree, DoseEdge).
• Demonstrated ability to multitask, prioritize, and lead in a fast-paced environment.
• Excellent verbal and written communication skills to interface with clinicians, patients, and interdisciplinary teams.
Physical Requirements
• May require standing for extended periods and lifting up to 25 lbs.
• Must be able to work occasional evenings, weekends, and on-call rotations as needed for patient care continuity.
