    BayCare is currently in search of our newest **Occupational Therapist - Pool Team Member** who is passionate about providing outstanding customer service to our community. Our Morton Plant Rehabilitation Center located on the campus of Morton Plant Hospital is proud to be recognized as a 5-star rated facility by The Centers for Medicare and Medicaid Services (CMS).

    Occupational Therapist - Pool/PRN Responsibilities:

    + Conduct comprehensive occupational therapy evaluations to assess residents' physical, cognitive, and psychosocial functioning.

    + Develop individualized treatment plans based on assessment findings, resident goals, and evidence-based practice.

    + Provide skilled occupational therapy services to residents, including therapeutic exercises, functional activities, adaptive equipment training, and environmental modifications.

    + Monitor residents' progress and modify treatment plans as necessary to achieve optimal outcomes.

    + Educate residents, families, and caregivers on therapeutic techniques, safety strategies, and home exercise programs to support continuity of care.

    + Document occupational therapy evaluations, progress notes, and discharge summaries in accordance with regulatory standards and facility policies.

    + Collaborate with interdisciplinary team members, including physical therapists, speech-language pathologists, nurses, social workers, and physicians, to ensure comprehensive care coordination.

    + Participate in interdisciplinary team meetings, care conferences, and discharge planning to facilitate seamless transitions of care.

    + Adhere to ethical and professional standards of practice, including maintaining confidentiality and respecting residents' autonomy and dignity.

    Requirements:

    + BLS (Basic Life Support)

    + OT (Occupational Therapy)

    + Or OT (Occupational Therapy) Eligible to obtain

    + Required Bachelor's Occupational Therapy

    + Preferred 1-year Occupational Therapy
