    BayCare Health Systems is currently seeking a **Discharge Planner 1** to join our outstanding and compassionate team at the Behavioral Health Center!

     

    + **Facility:** Behavioral Health Center

    + **Location:** Tampa, FL

    + **Status:** Full time, Hourly

    + **Shift Hours:** 8:00 am- 4:30 pm

    + **Weekend Work:** Occasional

    + **On Call:** No

    The **Discharge Planner I** responsibilities include:

    + Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.

    + Possesses a clear understanding of discharge and transition planning and linkage with community resources.

    + Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.

    + Provides support as active member of the treatment team.

    + Will perform other duties as assigned by supervisor.

     

    _**Travel may be required.**_

    Required Certification:

    + BLS (Basic Life Support)

    Required Experience:

    + Behavioral Health - 1 year

    + Child and Adolescent - 2 years (SIPP Only)

    + Acute Care - Preferred 1 year

    Required Education:

    + Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing

    + Master's in human services - Preferred
