    Pharmacy Services Coordinator / Clinical Pharmacy Liaison

     

    Reports To: Director of Nursing / Administrator

     

    Department: Clinical Services

     

    FLSA Status: Exempt / Full-Time

     

    Position Summary: The Pharmacy Services Coordinator / Clinical Pharmacy Liaison acts as the primary point of contact between Personal Healthcare and its contracted pharmacy. This position ensures that resident medications are reviewed regularly, therapeutic alternatives are appropriately considered, and communication among the pharmacy, nursing staff, and facility leadership remains efficient and resident-focused. The Coordinator supports high standards of pharmaceutical care while contributing to the organization’s commitment to quality, safety, and clinical excellence.

    Essential Duties and Responsibilities:

    + Coordinate ongoing reviews of resident medication profiles in partnership with the consulting pharmacist.

    + Identify opportunities for therapeutic interchange and evidence-based optimization of medication regimens.

    + Facilitate communication between facility staff and pharmacy to resolve medication discrepancies or supply issues.

    + Monitor and report trends in medication utilization and outcomes.

    + Participate in clinical and quality improvement meetings.

    + Maintain awareness of formulary changes, medication guidelines, and regulatory requirements.

    + Educate staff on medication-related processes, new drugs, and best practices as appropriate.

    + Support compliance with all state, federal, and facility policies related to pharmaceutical care.

    Qualifications:

    + Associate’s or Bachelor’s degree preferred; equivalent healthcare experience considered.

    + Prior experience in pharmacy operations, healthcare administration, or medication management preferred.

    + Knowledge of long-term care regulations and medication standards a plus.

    + Strong organizational, analytical, and interpersonal skills.

    + Proficiency in EMR systems and basic computer applications.
