    Job Summary and Qualifications

     

    Maintains and demonstrates clinical expertise in the area of Sepsis standards of care. Reviews best practices and shares information with all staff and Medical Staff.

     

    Reviews up-to-date evidence based literature and shares information with the staff and medical staff.

     

    Collaboratively works with the Medical Staff and interdisciplinary teams to develop and implement clinical pathways, order sets and algorithms.

     

    Collaboratively works with all staff and Medical Staff to ensure Sepsis Core Measures are implemented timely.

     

    Reviews Sepsis polices annually

     

    Revises based on up-to-date literature and change in standards of care.

     

    Responds to Code Sepsis calls to facilitate the completion of all requirements.

     

    Provides an analysis of each Code Sepsis and provides opportunities for improvement to all staff and medical staff timely.

     

    Chairs the Interdisciplinary Sepsis Committee. Serves as the leader of Sepsis process improvement initiatives.

     

    Participates in monthly Sepsis Capital division calls.

     

    Monitors and performs data collection for all Sepsis patients.

     

    Aggregates and analyzes data for identification of opportunities for improvement.

     

    Directs development and implementation of patient/family Sepsis education.

     

    Directs education for EMS on Sepsis initiatives and provides feedback as needed for process improvement.

     

    Provides orientation of the Medical Staff on Sepsis measures and expectations.

     

    Performs community education per TJC standards.

     

    Performs Sepsis drills monthly.

     

    Communicates with leaders regarding service or personnel issues.

     

    Demonstrates a commitment to personal professional development by seeking education opportunities to expand current knowledge.

     

    Adheres to all hospital policy and procedures.

     

    Works closely with the Quality Manager and VP Completes annual Code of Conduct · Demonstrates understanding of the HCA Code of Conduct

     

    Understands and satisfies the needs of the patient population appropriate to the age of the patient served.

    Essential education/licensure and experience:

    Current NH nursing license

     

    Associates degree required. Bachelor’s degree preferred.

     

    3-5 years of hospital experience

     

    Previous Sepsis experience preferred.

     

    Current BLS certification
