Job Summary and Qualifications
Maintains and demonstrates clinical expertise in the area of Stroke standards of care. Performs as the Stroke Expert and Performance Improvement Coordinator of all stroke initiatives.
Reviews best practices and shares information with all staff and Medical Staff. ·
Reviews up-to-date evidence based literature and shares information with the staff and Medical Staff.
Performs as The Joint Commission Certification Coordinator and ensures all standards are met.
Participates in daily PI coordinator huddle.
Completes at least 8 hours of Stroke education annually
Collaboratively works with the Medical Staff and interdisciplinary teams to develop and implement clinical pathways, order sets and algorithms.
Collaboratively works with all staff and medical staff to ensure Stroke Core Measures are implemented timely.
Reviews Stroke polices annually. Revises based on up-to-date literature and change in standards of care.
Ensures that all Stroke Core Measures/data are abstracted accurately and timely.
Performs an analysis of each Code Stroke and send out opportunities for improvement timely, including physician opportunities for improvement.
Responds to Code Strokes as appropriate to facilitate the completion of all requirements.
Chairs the Interdisciplinary Stroke Committee. Participates in monthly Stroke Capital division calls.
Performs data submission to Get with the Guidelines
Performs Stroke community twice yearly.
Performs Stroke drills monthly.
Responsible for Stroke post discharge follow-up calls.
Directs development and implementation of patient/family Stroke education.
Directs education for EMS on Stroke initiatives and provides feedback as needed for process improvement.
Provides orientation of the Medical Staff Stroke measures and expectations.
Communicates with leaders regarding service or personnel issues.
Demonstrates a commitment to personal professional development by seeking education opportunities to expand current knowledge.
Adheres to all hospital policy and procedures.
Completes annual Code of Conduct
Demonstrates understanding of the HCA Code of Conduct
Understands and satisfies the needs of the patient population appropriate to the age of the patient served.
Essential education/licensure and experience:
Current New Hampshire nursing license
Associates degree required. Bachelor’s degree preferred.
3-5 years of hospital experience
Previous Stroke experience preferred.
Current BLS certification
