The Nurse Manager is responsible for overseeing the performance and regulatory compliance of nursing staff across assigned home care branches in the Northern Oregon region!
The Nurse Manager ensures the delivery of high-quality, compliant, and compassionate care by supervising nursing personnel, maintaining adherence to Oregon Administrative Rules (OARs) Oregon State Board of Nursing, and company policies, supporting care planning and medication services, and fostering collaboration across interdisciplinary teams. They work closely with Branch Managers to ensure alignment between nursing services and branch operations, supporting overall client satisfaction and quality of service.
This position reports directly to the Director of Operations and serves as a key leader within the regional home care structure.
Duties and responsibilities
Home Care Oversight & Quality Assurance
+ Ensure all nursing services are delivered in strict compliance with Oregon Administrative Rules (OARs), company policies, and applicable licensing regulations.
+ Oversee timely completion of client assessments, care plans, and service onboarding documentation.
+ Supervise compliance with RN/LPN requirements, including verification that RN evaluations are conducted every 90 days for all clients receiving Medication Administration services.
+ Train and guide nursing staff on the distinctions between Medication Reminding, Medication Assistance, and Medication Administration, ensuring accurate classification and documentation.
+ Perform regular audits of medication-related documentation and client records to ensure accuracy, ongoing compliance, and to minimize the risk of regulatory citations.
+ Collaborate with branch staff to ensure clients receive services appropriate to their needs and as defined by state regulations.
+ Monitor and support nursing staff to ensure delegated tasks are performed in accordance with OARs, including proper assessment, documentation, training, and supervision.
+ Ensure proper oversight for clients receiving Medication Reminding, confirming eligibility criteria and preventing misclassification.
+ Update medication-related policies, procedures, and forms as needed.
+ May be required to maintain administrator status on an Oregon OHA license.
Team Leadership & Development
+ Supervise and support RNs, LPNs, and other nursing staff across multiple branches.
+ Conduct regular check-ins, case conferences, and performance evaluations.
+ Complete annual performance evaluations for all nurses under supervision, providing feedback and supporting development.
+ Ensure accurate and timely completion of all nursing staff timecards and PTO approvals.
+ Provide or assign RN coverage for branches without nurse support.
+ Identify training needs and facilitate education or mentoring.
+ Partner with Branch Managers, Caregiver Managers (CGMs), and Client Care Supervisors (CCSs) to resolve care-related concerns and support positive client outcomes.
Care Coordination & Client Engagement
+ Collaborate with branch teams to ensure timely starts of care, reassessments, and transitions.
+ Participate in client onboarding and act as a nursing liaison for complex or high-risk cases.
+ Address escalated concerns related to nursing care, ensuring timely resolution and clear communication with families and referral sources.
Compliance & Documentation
+ Ensure strict adherence to OARs regarding medication services: Medication Reminding, Medication Assistance, and Medication Administration.
+ Oversee accurate classification of medication-related services in care plans to ensure compliance and proper delivery.
+ Monitor that RN-delivered Medication Services are completed within two weeks of receiving the request.
+ Ensure consistent maintenance of required documentation, including signed physician orders and MARs, in alignment with OAR 333-536-0075(3).
+ Ensure weekly medication training sessions are being conducted by nurses in their respective branches.
+ Be available to support and provide documentation during audits, compliance reviews, or licensing surveys.
Collaboration & Communication
+ Participate in regional leadership and operations calls.
+ Provide nursing updates and recommendations to Branch Managers and the Director of Operations.
+ Collaborate with HR and recruitment teams to onboard and retain nursing staff.
+ Perform other related duties as assigned by leadership.
Qualifications
+ Active Oregon RN license in good standing.
+ Ability to pass both state and national background checks.
+ Personal vehicle, valid driver's license, proof of auto insurance, and a clean driving record.
+ Use their personal cellular hotspot when necessary.
+ Minimum of 1 year of experience as a nurse in home care, or at least 1 year of experience in a skilled nursing facility (SNF), ICU, or Med/Surg unit.
Working conditions:
The Nurse Manager will spend 85% of their time in an office environment indoors. This will require time both spent at a computer station for office work and standing. The additional 15% of their time will be in various client homes performing observations.
This position will require regular travel to all Oregon branches, it's expected travel from home location will be approximately 50% of the time.
This position will have administrative supervision.
