    Description

    TheOutreach Worker/Case Managersupports the Undetectables Program, a Public Health Solutions-funded initiative at GMHC that promotes the U=U (Undetectable = Untransmittable) message. The position focuses on engaging and supporting clients living with HIV to achieve and maintain viral suppression through outreach, education, and comprehensive case management services.

     

    The Outreach Worker/Case Manager conducts targeted outreach and in-reach to identify and enroll eligible clients, provides treatment adherence counseling and modified directly observed therapy (mDOT), facilitates care/service planning, and coordinates referrals to medical and psychosocial resources. The role also includes distribution of client incentives, documentation in the electronic medical record (EMR), and participation in case conferences to ensure coordinated care.

     

    Essential Job Functions

    The Outreach Worker/Case Manager will be responsible for the following duties:

    Outreach and Engagement

     

    + Conduct in-reach within GMHC, and outreach at local health fairs, community partner sites, and targeted mobile testing van events.

    + Recruit, screen, and enroll eligible clients into the Undetectables Program.

    + Collaborate with internal departments and external partners to strengthen referral pathways and community linkages.

     

    Case Management and Support Services

     

    + Develop individualized care/service plans in collaboration with clients to promote self-efficacy and treatment adherence.

    + Provide adherence counseling and modified directly observed therapy (mDOT) for clients who require additional support.

    + Participate in interdisciplinary case conferences with medical providers and other care team members.

    + Refer clients to medical, preventive, and psychosocial services as appropriate.

     

    Program Operations and Documentation

     

    + Document all client interactions in the EMR in accordance with GMHC and funder requirements.

    + Distribute client incentives to eligible participants and maintain accurate tracking records.

    + Assist in organizing and participating in community outreach events and special projects.

    + Perform other related duties as assigned by the supervisor.

    Requirements

    Special Skills and Knowledge

    Required:

    + Strong verbal, written, and interpersonal communication skills.

    + Proficiency with Microsoft Office Suite (Word, Excel, Outlook) and familiarity with electronic records systems.

    + Knowledge of HIV/AIDS, sexual health education, PrEP/PEP, and related public health issues.

    + Ability to organize, manage workload independently, and travel throughout the five boroughs.

    + Commitment to working collaboratively in a multidisciplinary team environment.

    Preferred:

    + Bilingual English/Spanish.

    + Experience providing outreach or case management services to people living with HIV and/or LGBTQ+ communities.

     

    Education and Certification

     

    + High School Diploma or educational equivalent required.

    + Associate or Bachelor’s Degree in a relevant field preferred.

    + A combination of education and a minimum of four years’ experience providing outreach and case management services to hard-to-reach populations will also be considered.

     

    Technical Skills

     

    + Proficient in Microsoft Office Suite (Word, Excel, Outlook) and video conferencing platforms.

    + Experience with data entry and documentation in electronic medical or case management systems (e.g., eCOMPAS, AWARDS, or comparable platforms).

    + Understanding of confidentiality requirements and HIPAA compliance.

     

    Work Environment & Schedule

     

    This is a Hybrid position that includes a combination of on-site and remote work, determined by program needs and supervisor discretion. Key expectations include:

     

    + In-office presence for team meetings, cross-departmental collaboration, supervision, and other in-person requirements.

    + Remote work flexibility for documentation, reporting, and administrative responsibilities.

    + Conduct field-based outreach, including attendance at local health fairs, community partner sites, and targeted mobile testing van events

    + Must adhere to all workplace policies, and health and safety regulations at all times

    + Maintain availability for late afternoon, evening, and occasional weekend hours to meet program and community needs.

    + Participation in the annual AIDS Walk New York event is mandatory for all GMHC staff.

     

    Physical Demands

     

    + Travel and Mobility: Frequent travel across the five boroughs of New York City using public transportation.

    + Physical Activity:Employee is routinely required to sit, stand, walk, and navigate stairs while performing outreach or client support activities.

    + Communication: Ability to engage in frequent verbal and written communication, both in-person and virtually.

    + Documentation:Consistent focus and mental concentration are required to record detailed information accurately, analyze data, solve complex problems, and develop effective solutions within tight deadlines.

    Salary Description

    60,000 - 65,000 (DOE)
