The Position
Job Appointment: Full time, regular.
Work Schedule: Employees in this position will work four (4) 10-hour days with three (3) days off a week. Shifts include days, afternoons, and nights, including holidays. Ability to work any shift and on any day of the week is essential. During training, your shift may change depending on the availability of the trainers. After completing probation, you will be able to bid on an open shift based on your seniority.

About Portland Street Response (PSR):
Portland Street Response is a compassionate, trauma-informed first response agency. PSR is dispatched by the Bureau of Emergency Communications (BOEC) to address mental and behavioral health crises, as well as non-emergency situations involving vulnerable populations.

Position Summary
Under general direction of their supervisor, the Community Health Worker (CHW) serves on the multi-disciplinary PSR Aftercare team along with Peer Support Specialists (PSS). The role of the CHW is to connect with clients after 911 calls have been resolved. Our Crisis response teams connect at the time of the 911 call and will refer clients needing more assistance to the Aftercare team. The CHWs and PSSs work closely to complete all tasks associated with follow-up crisis stabilization services, as well as various other administrative tasks.
Responsibilities include developing individualized care plans, which may include VI-SPDAT/MSST screenings, OHP application support, shelter referrals, housing readiness preparation, sobriety and harm reduction support, and advocacy with agency partners such as Aging and Disability Services and health clinics.
Other responsibilities include:
+ Coordinating care for clients/individuals referred from the PSR crisis response team. Clients are people referred by the Mental Health Responder team, who are experiencing mental health and behavioral health crises in the community.
+ Providing individual support, such as, creating a care plan/SMART goals, advocacy, and assisting clients with connecting to resources/referrals in order to support their stabilization.
+ Assisting clients/individuals in applying for Oregon Health Plan and other benefits support services such as shelter/housing services, health and wellness programs, Aging & Disability services.
+ Field Work, including off-site visits with the clients, such as accompanying the client to their appointments, dropping off food boxes, filling out applications with client, and at home-shelter-camp-street visits.
+ Documenting services with clients. CHW’s will document their client encounters, and services provided, care plan/SMART goals, referrals to community partners, and other relative information. They will accurately chart this information in order to support statistical data.
+ Developing and maintaining effective working relationships with communities served and community partners.
+ Coordinating services for clients in collaboration with members of the care team including complex system navigation and accompanying clients to specialty appointments, as needed.
+ Supporting individuals connecting with external health/social services agencies and act as an advocate and support service.
+ Navigating working with a multi-disciplinary team.
+ Establishing effective working relationships and collaborating with external community service providers.

The following minimum qualifications are required for this position:

+ Experience as a first responder or frontline community-based crisis responder.
+ Experience working effectively in multi/intra-disciplinary settings with other first responders, health and community agencies.
+ Bi-lingual fluency preferred.
