**Job Description** The following specifications cover RN qualifications and duties for Capital District DDSOO community sites throughout Albany, Rensselaer, Saratoga, Schenectady, Warren and Washington counties.
Provide skilled nursing care to patients requiring short timeframe professional nursing services.
Key Responsibilities:
Conduct weekly home visits; assess health needs and provide direct nursing care
Monitor health status and maintain clear, timely documentation
Coordinate and track medical appointments and follow-ups
Medication administration and tube feeding via gastrostomy or jejunal tube observation
Oversee medication administration; review MARs and ensure compliance with orders
Supervise and recertify AMAPs and LPNs
Participate in treatment planning, case reviews, and team meetings
Train staff on nursing care plans, health conditions, and infection control
Maintain compliance with Nursing Policies & Procedures and regulatory standards
Provide emergency care (CPR) and short-term skilled nursing services as needed
RN’s may need to step in and perform LPN duties when needed.
