    Summary:

    + In collaboration with the Clinician team member, the Case Manager (Mental Health Worker) provides community-based therapeutic services and case management for children ages 4-19 with severe emotional disturbance (SED) who are at high risk of hospitalization, juvenile justice involvement and foster care placement. The Case Manager also provides case management and therapeutic services for their caregivers and family members and collaborates with Clinician in family therapy sessions. Services include meeting and liaising with school personnel, mental health treaters, medical providers, DHS/CUA, and other providers. Travel to client homes, schools, hospitals, residential treatment facilities (RTFs) and other community sites is required. Case managers also regularly provide crisis response services to support families in de-escalating and achieving safety. Each PHIICAPS team typically provides service to eight children and their families averaging of 4-5 hours working with each family every week. Paid after-hours on-call responsibilities are included in the PHIICAPS model. The standard schedule for Case Managers is 10a-6p, M-F, but flexibility is required to meet clients’ scheduling and safety needs outside of those hours.

    Responsibilities:

    + Adheres to Compliance standards in service provision and professional development as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer and the TCM Program.Completes, monitors and maintains as current all required Service Documentation appropriate to Targeted Case Management, Hall-Mercer Outpatient services, TCM Program specific services and any ancillary services provided.Ensures TCM services are provided with attention to assigned caseload as well as in complement to and in cooperation with TCM Program specific requirements for and facility-wide expectation of Team-Oriented service provision.Submission of Service Documentation forms for billing, regulatory and monitoring purposes occurs as mandated by the TCM program and demonstrate the required productivity of TCM service provision units.Completes comprehensive Service Planning as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer, the TCM Program and regulatory agencies.Provides TCM Services as mandated by the Office of Mental Health, the Targeted Case Management Unit, Hall-Mercer, the TCM Program and regulatory agencies.

    Credentials:

    Education or Equivalent Experience:

    + Bachelor of Arts or Science (Required)

    + Education Specialization:Equivalent Experience:•And 2+ years MH/MR Experience

    + H.S. Diploma/GED (Required)

    + Education Specialization:At Least 15 College Credits in Human ServicesEquivalent Experience:•And 4+ years MH/MR Experience
