    JOB OVERVIEW

    The Health and Wellness Clerk is responsible for coordinating nursing and clinical activities within Wellness, ensuring a welcoming and responsive environment that prioritizes the well-being and safety of all residents. This role focuses on supporting clinical staff, coordinating resident healthcare needs, providing preventative and wellness services, through administrative, clerical, and team member scheduling support. The Health and Wellness Clerk is responsible for greeting all at wellness and managing both external and internal calls and taking and communicating messages. The Health and Wellness Clerk works closely with residents, families, and healthcare providers to ensure optimal care and quality of life within the community.

    RESPONSIBILITIES & QUALIFICATIONS

    Essential Duties

The unique responsibilities for this role include but are not limited to the essential functions listed as follows:

     

    + Support the overall operations of the Wellness clinic.

    + Coordinate the health and safety of residents through collaboration with third party services and the management team.

    + Participate in the coordination and communication of resident related updates with applicable parties and key stakeholders.

    + Collaborate with community leadership team.

    + Participate in community events and meetings as directed by Executive Director.

    + Order labs and X-rays as ordered by community physician.

    + Support the residents and coordinate with community physician/or resident’s primary physician, follow-up (Transfer to Hospital, schedule / same day or future appointment) with physician.

    + Coordinate with external pharmacy for vaccination clinics.

    + Participate in resident discharge planning as needed.

    + Coordinate services with external providers for residents including audiologist, podiatrist, dentist, and rehabilitation etc.

    + Support the wellness department scheduling including Medication Technician and Licensed Practical Nurse (LPN) shifts.

    + Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

    + Perform other duties as assigned.

    Experience and Qualifications

     

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.

     

    + Basic Life Support (BLS) certification; additional certifications in geriatric or community health nursing are desirable

    + Familiarity with electronic health record (EHR) systems

    + High School degree/GED

    + Proven customer service experience and skills

    + Computer proficiency (Word, Excel, Outlook, etc.)

    + Office Equipment Operation - printers, scanners, phones, tablets

    Selected candidates will be offered competitive compensation based on geographic location of community/office, skills, experience, qualifications, and certifications/licenses (where applicable).
