+ $2,500 sign-on bonus (external candidates)

Additional Preferred Requirements
+ At least 5 years of hospital-based RN experience
+ Prior Case Management experience
+ Proficient in EPIC
Overview
Since 1937, **Alton Memorial Hospital** has cared for residents in Alton, Illinois, and the surrounding communities in a five-county area. Starting as a land gift to the community from the Smith family, Alton Memorial today is a full-service acute care hospital. Alton Memorial offers patients a variety of inpatient and outpatient services, including surgery services, medical imaging, interventional and diagnostic heart services, cancer care, rehabilitation, 24-hour emergency care, ambulance services and more.
Case Management, Utilization Review, Social Services: Provides admission and continued stay reviews on all patients for the purpose of compliance and the proper utilization of resources. Provides social service resources to patient's and their families.
Preferred Qualifications
Role Purpose
Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
Responsibilities
+ Assesses, coordinates, and evaluates each patient's plan of care.
+ Establishes, continuously evaluates, and documents an individualized discharge plan.
+ Ensures clinical documentation and cost-effective use of hospital resources.
Minimum Requirements
Education
+ Nursing Diploma/Associate's - Nursing
Experience
+ <2 years
Supervisor Experience
+ No Experience
Licenses & Certifications
+ RN
