Job Description
The Case Manager I plays a crucial role in supporting the program by providing comprehensive case management services. The primary objective is to assist patients in obtaining income, insurance, and housing. This position involves addressing co-occurring disorders, such as mental health and substance abuse, with the aim of enhancing the patient's current functioning level and skill acquisition. The Case Manager will engage in assessment, facilitate access to treatment including medical care with a primary care provider, and support rehabilitation and goal achievement.
Responsibilities
+ Meet with each patient individually to conduct an intake needs assessment.
+ Manage a minimum caseload of 50 patients.
+ Write and enter progress notes daily into EHR systems to maintain patient files.
+ Incorporate a patient-focused approach to ensure the delivery of measurable outcomes.
+ Perform 30% of duties in the office and 70% traveling to conduct in-person member visits.
+ Maintain a daily 80% utilization of billable patient services.
+ Manage work schedule when meeting with members.
+ Transport patients to scheduled appointments and community resources.
+ Participate in care coordination meetings with various partners as deemed appropriate.
+ Advocate for patients in obtaining identification, housing, benefits, services, and entitlements from governmental and private agencies.
Essential Skills
+ High School Diploma or GED required.
+ Lived experience in serving unhoused, seriously mentally ill, substance use disorder, and/or chronically ill patients.
+ Ability to work collaboratively in a team environment, respecting diverse perspectives and experiences.
+ Maintain appropriate roles and boundaries among patients, peers, and partners/stakeholders.
+ Function effectively in a fast-paced environment.
+ Commitment to providing services for the underserved.
Additional Skills & Qualifications
+ Associates Degree preferred.
+ Medical assistant experience, familiarity with charts, EMR, and healthcare.
+ Knowledge of medical terminology and behavioral health.
+ Experience with systems such as NexGen and patient coordination.
Work Environment
The role operates across Sacramento and Placer County and follows a Monday to Friday day shift schedule. Employees are required to spend 30% of their time in the office and 70% traveling for in-person member visits. The position offers benefits such as medical, dental, and vacation. The pay range for this position is $50000.00 - $50000.00/yr.
