This is a statewide position and will report to their nearest DHW location.
This position may be eligible for limited telework after successful completion of probation and necessary training. Approval to telecommute is not guaranteed and subject to termination at any time.
The Idaho Department of Health & Welfare is a drug-free workplace. EXAMPLE OF DUTIES:
+ Conduct in-home level of care assessments to determine eligibility for Personal Care Services (PCS), Aged & Disabled (A&D) Waiver programs.
+ Observe care delivery and conduct interviews to ensure services meet state and federal standards.
+ Explain available services, and complete comprehensive evaluations to help individuals access Home and Community-Based Services (HCBS).
+ Serve as a trusted resource for participants, families, and providers.
+ Answer questions about HCBS and supplemental services, assist with provider-related concerns, clarifying service authorizations and eligibility.
+ Support the team and maintain program integrity through documentation and correspondence.
+ Participation in staff meetings and general office duties that keep operations running smoothly.
+ Conducting regulatory compliance reviews through client sampling.
+ Assess clients' health , emotional, and rehabilitative needs.
+ Applying critical thinking and clinical judgement to evaluate care quality, writing clear, concise, and legally sound reports.
