    Job Summary and Responsibilities

     

    The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team

    Job Requirements

    + Two (2) years LVN Experience within the last 5 years

    + Recent experience as an LVN in a Home Health or Hospice Agency - preferred.

    Licensure:

    + Currently licensed as a LVN in California

    + Current California driver's license, possession of automobile insurance, & access to an operating motor vehicle.

    + Current CPR/BLS card approved through the American Heart Association.

    Pay Range

    $43.12 - $48.63 /hour
