    Summary The Home Based Primary Care (HBPC) Program Director at the GS-13 Advanced Practice Level demonstrates advanced practice skills and judgment. The HBPC Program Director is licensed by a state to independently practice social work at the master's level and coordinates/directs the administrative and clinical aspects of a major specialty treatment program (Home Based Primary Care). The Incumbent directs the clinical services offered by the program to ensure that the program is in compliance with all VHA and Joint Commission Standards for Accreditation of Home Care as well as VISN and VACO priorities. The incumbent assists the team in developing and implementing local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, staff safety, and implementation of National Patient Safety Goals. The incumbent is responsible for the reporting mechanisms for patient satisfaction, emergency management, and infection control to the appropriate committees. The incumbent is responsible for managing human resources for the HBPC program including: selecting qualified staff in collaboration with the HBPC Medical Director, orienting and mentoring the staff, and promoting staff development and professional growth to ensure competency. The incumbent manages and tracks program resources as well as monitoring and assessing staff productivity. The duties of the Program Director involve collaborating with the Office of Information Technology to ensure accurate reporting of HBPC clinical, administrative and workload data. The incumbent demonstrates mastery of advanced management and organizational principles and practices along with a comprehensive knowledge of planning, programming and budgetary regulations, guidelines and processes. The incumbent possesses thorough knowledge of home health care, primary care, agency goals and objectives, missions, and organizational relationships plus knowledge of VA requirements and other standards or codes related to patient care activities. The incumbent has expert ability to plan and implement short-term and long-range goals in accordance with VHA, VACO and Medical Center directives and policies. The incumbent analyzes and assesses the potential for growth of the HBPC program, in order to meet the needs of the identified veteran population and in accordance with National and VISN performance measures. The incumbent works with the various stakeholders and promotes the required Request for Funding with all necessary supportive data; when such initiatives are approved, the incumbent then implements and coordinates the expansion, which can occur both at the primary site as well as in satellite settings. As Rural Health and Indian Health initiatives develop, incumbent is responsible for developing, implementing and directing the HBPC program at the main site as well as potential expansion to the Community Based Outpatient Clinics (CBOCs). The incumbent displays expert knowledge of evaluative methods to continually and systematically analyze operations. The incumbent provides reviews and provides analysis of complex data and reports, including OPs reports, Austin DMS COIN 157 reports, Electronic Waiting Lists, KLF, HB Information System, DSS reports and hospital utilization reports to develop strategic plans and program activities. The incumbent is responsible for ensuring staff compliance with ORYX measures, clinical reminders, and performance improvement measures. The incumbent may supervise the daily work of the Program Support Assistant and the social work staff assigned to the program. For interdisciplinary staff matrixed to the HBPC Program (Registered Dieticians, Rehabilitation Therapists, Psychologists, Pharmacist) the incumbent provides programmatic direction, advice, counsel, and instructions to individual professional staff members on clinical and administrative matters. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. If not fully licensed at an acceptable independent social work practice level, a candidate has 3 years to become fully licensed as a condition of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Knowledge of organizational structure and impact on program operations. Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-185 social work series and may result in termination of employment.
