    Dietitian Details:

    Kinship Health is searchingfor a Registered Dietitian that wants to make a difference and serve theelderly population.

     

    The Registered Dietitian isresponsible for providing comprehensive assessment and nutrition treatment andcoordination of 24-hour care for PACE participants as part of theinterdisciplinary team. Plans, organizes, and conducts a comprehensive programof individual and group nutrition services, including the provision of thenutrition needs of the participant, clinical guidance, nutrition education, andcounseling to meet normal and therapeutic needs.

    KEY RESPONSIBILITIES

    + Performs initial face-to-face assessment and evaluates the nutritional status of individual participants utilizing nutritional guidelines, professional knowledge, and interviewing skills to provide quality care. Performs annual assessments and reassessments as required. ? Assessments may be conducted in the PACE Center or in the participant ’ s home.

    + Evaluate participants’ physical condition and calculate therapeutic diets to meet the specific requirements of individuals. Makes recommendations to the team on the plan of care and status of participant progress.

    + Provides clinical recommendations and creates orders that will generate authorizations to reflect the individualized needs of each participant under the approval of the Primary Care Provider.

    + Maintains an up-to-date file of all participants at assigned locations to include their diet order, modifications, allergies, restrictions, and likes/dislikes.

    + Establishes nutritional plans on assessment of participant’s condition, monitors participant’s progress, and recommends/revises changes in nutritional plan for effective participant care.

    + Develops and implements the nutrition care plan to promote, restore, or maintain the participant’s optimum nutrition status, ensuring integration into the interdisciplinary team’s plan of care. Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of independence.

    + Documents change in the participant’s condition and detail care provided by completing all required PACE documentation and ensuring compliance with Agency standards and policies.

    + Monitors participant progress and outcomes through assessment of care provided, including the need for revision to the treatment plan. Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as needed.

    + Provides nutrition counseling to participants and/or caregivers utilizing educational materials, modified diets, and teaching aids to ensure participants' understanding of the overall plan of care and its implications.

    + Participates in PACE in-service education programs, quality reviews, and interdisciplinary team meetings as required. Consult with team members on hard-to-service cases.

    + Works with the Regional Medical Director to determine the nutritional adequacy of menus and the caloric and nutritional needs for the participant population.

    + Works with food service vendor to develop menus adequate to meet the caloric and nutritional needs of participants.

    + Acts as a liaison with other community agencies to gather and disseminate information on nutritional trends and changes.

    + Travels to participants’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilitates, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the participant.

    + Assists center caregivers by working with the center manager to recommend solutions regarding specific nutritional needs.

    + Conducts Dining/Meal Observation as needed.

    + Manages stock of regular food supplies for breakfast and snacks and places orders as needed.

    + Manages emergency food supply stock ensuring the PACE Center maintains no less than a24-hour supply of food and water.

    + Develops a 4-week meal plan for the PACE Center in conjunction with the catering vendor.

    + Monitors recorded refrigerator/freezer temperatures and recorded food temperatures to ensure safety and compliance.

    + Other duties as assigned.

    REQUIRED EDUCATION &EXPERIENCE

    + Bachelor's Degree from an accredited Dietetics program. ?

    + A minimum of one (1) years’ experience in clinical nutrition.

    + Must have one (1) year of experience with the frail or elderly population.

    + Ability to travel to participant homes, nursing homes, and sites/clinics.

    + Valid driver's license, current insurance.

    + Knowledge of PACE regulations is preferred.

    + Serv-Safe Manager Certification required or ability to obtain prior to hire
